Virtual Family Practice Nurse Practitioner – Temporary, TX, LA Licensed

• Conduct virtual adult and/or pediatric wellness visits, health assessments, and related healthcare services
• Review caseloads and prepare technology before starting the day
• Navigate daily schedules using modern technology
• Partner with clinical in-home team members visiting patients’ homes
• Connect with patients and collect relevant information
• Perform health risk assessments, medication reviews, cognitive screenings, and patient health education
• Evaluate vital signs with Sprinters, arrange blood draws, and complete necessary patient‑care tasks
• Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources, and documentation

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in TX, LA, or OK
• Prior experience performing wellness visits
• High levels of professionalism, integrity, accountability, confidentiality, care, and compassion
• Willingness to perform last-minute problem solving and out-of-the-box thinking
• Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA-secure messaging apps
• Strong written and verbal communication skills
• Ability to work independently or in a team environment
• Pass national background check and valid clinical license search
• Current prescriptive authority in the state(s) where licensed to practice as a Nurse Practitioner

Demonstrates expertise in conducting virtual wellness visits and health assessments while ensuring high standards of patient care and confidentiality. Proficient in care coordination, medication management, and utilizing technology for effective patient communication and documentation.
