RN Hospice Admissions
This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.
Position Responsibilities:- Coordinate and oversee all aspects of new patient admissions
- Collaborate with patient care teams to support seamless communication and continuity of care
- Educate patients and families regarding condition, needs, and care
- Perform head-to-toe physical assessments
- Complete all required documentation accurately and on time
- Current Registered Nurse license
- Current driver's license
- Current CPR certification
- Two years of Registered Nurse experience in a clinical care setting
- One year of full-time hospice experience
- Reliable transportation and valid automobile liability insurance
- Health Care Plan (Medical, Dental & Vision)
