Home Health LPN​/Visit

Full Time Licensed Practical Nurse based in Independence, MO, with occasional travel to Overland Park, KS. The role provides direct patient care under RN supervision, administers medications, monitors patients, documents in Matrix EMR, and collaborates with interdisciplinary teams. Candidates must hold Missouri and Kansas LPN licenses and have wound and catheter care experience.

• Provide direct patient care under RN supervision

• Administer medications and treatments per physician orders and facility protocols

• Monitor patient conditions and report changes to the clinical team

• Accurately document patient information, treatments, and care plans using Matrix EMR

• Assist with admissions, discharges, and ongoing patient assessments

• Collaborate with interdisciplinary teams to ensure quality outcomes

• Educate patients and families about treatment plans and care instructions

• Maintain compliance with state, federal, and facility regulations

• Deliver compassionate, patient-centered care in a fast-paced setting

• Graduate of an accredited Licensed Practical Nursing (LPN) program

• Active Missouri LPN license required

• Compact nursing license or active Missouri and Kansas LPN license required

• Wound care and catheter care experience required

• Strong communication, organizational, and clinical assessment skills

• Ability to work independently and collaboratively within a healthcare team

• Minimum of 1 year of LPN experience

• Current CPR/BLS certification

• Experience with Matrix EMR
