About the Role
The Social Worker is responsible for promoting the spiritual, emotional, and mental well‑being of the patients and families served. This role provides quality social work services through assessment of patient needs, identification of discharge planning concerns, coordination of resources, and facilitation of safe and effective transitions of care. The Social Worker collaborates with patients, families, healthcare providers, and community agencies to support positive patient outcomes and continuity of care.

Your role as a Social Worker makes a meaningful impact by supporting the spiritual, emotional, and mental well‑being of patients and families during times of need. Through compassionate care, advocacy, discharge planning, and coordination of resources, you help ensure patients receive the support and services needed for safe transitions and improved overall outcomes.
Patient Care & Psychosocial Assessment- Conduct interviews with patients and family members to gather information regarding home environment, family relationships, health history, and psychosocial needs.
- Assess and evaluate patient information in relation to the medical treatment plan, available social service programs, and appropriate therapeutic resources.
- Assist patients and families in understanding, accepting, and following discharge and post‑hospital care plans.
- Participate in patient care planning conferences and interdisciplinary team discussions as required.
- Develop and coordinate discharge and post‑hospital care plans to support safe and effective transitions of care.
- Utilize community resources and social service agencies to assist patients in resuming community life or adapting to disabilities and ongoing care needs.
- Collaborate with hospital departments, healthcare providers, and community agencies involved in patient care and support services.
- Prepare, maintain, and update required case documentation in a timely and accurate manner.
- Maintain confidentiality of all patient and medical information in accordance with organizational policies, HIPAA regulations, and professional standards.
- Adhere to established policies, procedures, quality assurance standards, safety regulations, and environmental guidelines.
- Demonstrate behaviors consistent with the organization’s mission, values, and goals.
- Promote positive patient, family, and associate relationships through professional and compassionate interactions.
- Maintain dependable attendance, punctuality, and accountability in daily responsibilities.
- Maintain current knowledge of social work practices, state and federal regulations, and industry standards through continuing education and professional development activities.
- Attend weekly skilled care meetings and monthly leadership meetings as required.
- Perform additional duties and responsibilities as assigned by the Chief Nursing Officer.
- Bachelor of Science in Social Work (BSW) required.
- Current Kansas state Bachelor of Social Worker (LBSW) licensure required.
- Previous experience in a healthcare setting as a Social Worker preferred.
- Maintains continuing education requirements necessary to sustain active licensure and professional competency.
- Demonstrates compassion, empathy, and professionalism when working with patients, families, staff, and community partners.
- Possesses strong interpersonal and communication skills with the ability to build trusting and supportive relationships.
- Maintains confidentiality and exercises sound judgement in sensitive situations.
- Demonstrates adaptability, dependability, and the ability to remain calm and effective in stressful or emotional situations.
- Must be able to interact respectfully and effectively with individuals from all economic, educational, and cultural backgrounds.
