Aspirus Health in Rhinelander, WI is seeking a Social Worker – MSW to join our Hospice Homecare team covering Northern Wisconsin sites.
Under supervision, you will support patients and families facing end-of-life challenges, conduct psychosocial assessments, plan discharges, and coordinate with an interdisciplinary team.
Qualified candidates have an MSW from CSWE-accredited program, at least one year hospice or medical social work experience, and a valid Wisconsin or Michigan driver's license.
