We are seeking a Physical Therapist to provide care across outpatient, inpatient, swing bed, and long-term care settings. Approximately 70–75% of the caseload will be outpatient, with additional coverage supporting other rehabilitation services as needed.

• Evaluate patients and develop individualized treatment plans
• Provide physical therapy services across outpatient, inpatient, swing bed, and long-term care settings
• Monitor patient progress and modify treatment plans as appropriate
• Educate patients and caregivers on exercises, mobility, and injury prevention
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Maintain accurate and timely clinical documentation
  
  Participate in discharge planning and care coordination

• Graduate of an accredited Physical Therapy program
• Active Physical Therapist license or eligible for licensure
• Strong clinical assessment and treatment planning skills
• Ability to work independently and as part of a multidisciplinary team
