Occupational Therapist (OT) - SNF ​/ LTC - Contract

Job Summary

A great SNF / LTC is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a contract assignment!

• Provide direct patient care to assess medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients.
• Document and record the patient’s condition.
• Educate patients and families in an appropriate occupational therapy method.

• Graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or actively in the process of applying for licensure.
• Current CPR certification.
• Provide skilled occupational therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of occupational therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
