Citizens Health seeks a Community Health Worker for a full-time role in Colby, KS. The CHW will promote healthy aging, provide education on physical and mental health, social engagement, and access to community resources. You will work with the Care Team to address Alzheimer’s-related challenges and overall community health needs.
Location is Colby, with hours 8 AM to 5 PM, Monday through Friday. Strong communication and connection to the local community are essential for success in this role.
