Position Purpose

The Advanced Practice Registered Nurse (APRN) provides comprehensive, evidence-based, trauma-informed, and person-centered healthcare services through Kanza Mental Health and Guidance Center's Medical Services Department, with an initial emphasis on delivering care in partnership with and at local Northeast Kansas Multi-County Health Department offices.

The APRN functions as an advanced practice clinician providing assessment, diagnosis, treatment, medication management, health education, preventive care, care coordination, and follow-up services within the APRN's education, national certification, population focus, Kansas licensure, demonstrated competency, clinical privileges, organizational policies, and applicable federal and state law.

The APRN will provide and coordinate primary and preventive healthcare services with an emphasis on maternal and reproductive health, including prenatal and postpartum care; substance‑use disorder treatment and medication‑assisted treatment (MAT), including medication for opioid use disorder (MOUD); behavioral‑health integration; chronic disease management; and other medical and community health services identified by Kanza Mental Health and Guidance Center.

VI. Care Coordination and Referral Management
  • Coordinate care among individuals receiving services, family members or designated supports, nurses, medical assistants, behavioral‑health providers, pharmacies, laboratories, hospitals, specialists, primary care providers, Northeast Kansas Multi‑County Health Department personnel, and community organizations.
  • Review outside medical records, laboratory results, hospital records, medication lists, and other relevant clinical information.
  • Make appropriate referrals to primary care, specialty care, obstetric services, maternal‑fetal medicine, behavioral health, substance‑use treatment, emergency services, social services, and other resources.
  • Follow up on referrals, diagnostic studies, laboratory results, hospitalizations, emergency department visits, and other significant clinical events.
  • Assist individuals in navigating healthcare systems and accessing needed medical, behavioral‑health, social‑service, and community resources.
  • Develop and maintain collaborative relationships with local hospitals, pharmacies, healthcare providers, behavioral‑health organizations, Northeast Kansas Multi‑County Health Department, and other community partners.
  • Participate in multidisciplinary case consultation and care planning.
  • Promote continuity of care when individuals transition between Kanza Mental Health and Guidance Center, the Health Department, hospitals, primary care, specialty care, behavioral health, and community services.
VII. Clinical Documentation and Medical Records
  • Accurately and promptly document assessments, diagnoses, treatment plans, prescriptions, orders, procedures, referrals, communications, patient education, and follow-up activities in the electronic health record designated by Kanza.
  • Complete clinical documentation in accordance with Kanza requirements, professional standards, payer requirements, applicable laws and regulations, and any additional documentation requirements associated with collaborative programs.
  • Review laboratory and diagnostic results and document appropriate clinical follow‑up.
