Nurse Practitioner In-home Visits

Overview

We’re hiring board-certified nurse practitioner contractors to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Flexible hours, one‑on‑one patient relationships, and a strong mission to build trusted relationships to make people healthier.

• Conduct in‑home visits focused on a patient’s overall health and well‑being, identifying potential barriers to care.
• Perform medication and medical history reviews and complete a physical evaluation in the patient’s home.
• Follow provided patient‑specific forms; do not prescribe medicine, order lab tests, or alter existing treatment regimens.

Earn up to $2,500 to $4,000 per week (average 6–9 visits daily). Base rate is $80 per completed assessment.

• Increase per‑visit rate up to 135% or 160% with the Market Multiplier depending on the county.
• Earn additional pay through a pay‑per‑test model for qualifying visits.

• Board‑certified nurse practitioner.
• Valid state license.
• Medicaid license or willingness to enroll (with support) strongly preferred.

• Shifts available Monday‑Sunday; no nights or weekends required.
• Short‑term or long‑term assignments, pre‑scheduled in‑home visits.
• Independent contractor (1099), flexible schedule.
• Airfare, hotel, and rental car available where eligible (depending on the assignment).
• Malpractice coverage provided.
• Availability incentives based on weekly hours.
