** Job Description
** Scope of Position:  This position is responsible for the post discharge coordination of follow-up care provided to liver, kidney, and pancreas recipients after they have received a transplant.

Position Summary:

** Minimum Education Required
** Bachelor's Level Degree or equivalent combination of education and experience with a Major in nursing      
** Required Qualifications
** Graduate from an approved school of nursing. Current Ohio License. BSN preferred. Prior transplant experience preferred. Current Certified Clinical Transplant Coordinator (CCTC) certification preferred. Flexible, sensitive and adaptable to transplant activities.## Additional Information:##
