Your Role as the MDS Coordinator (LPN) Skilled Nursing

Americare Senior Living is seeking a detail‑oriented and clinically knowledgeable MDS Coordinator (LPN) to support accurate assessments, regulatory compliance, and quality resident outcomes within our skilled nursing community.

In this role, you’ll coordinate the MDS process in accordance with federal and state guidelines while also supporting direct resident care, interdisciplinary collaboration, and a safe, home‑like environment. Your work plays a critical role in care planning, reimbursement accuracy, survey readiness, and overall quality of life for residents.

Summary

The MDS Coordinator (LPN) is responsible for coordinating and completing the MDS process as required by federal and state regulations while ensuring accurate, timely, and compliant clinical documentation.

This role collaborates closely with the Director of Nursing, Administrator, and interdisciplinary team (IDT) to maintain facility standards, support inspections, and promote high‑quality resident care.

This Role May Also Be Known As

MDS Coordinator (LPN), MDS Nurse (LPN), Clinical Reimbursement Nurse (LPN), Assessment Coordinator, or Case Mix Nurse (LPN).

What You’ll Do
  • Coordinate and complete MDS assessments in accordance with federal and state guidelines
  • Ensure admission assessments, nursing notes, overall care plans, and required documentation are accurate and current per facility, ASI, and regulatory standards
  • Complete assigned MDS sections and required assessments, including but not limited to:
  • Skin, contracture, fall, incontinence, catheter, and side rail assessments
  • RAP summaries for urinary incontinence, falls, indwelling catheters, feeding tubes, dehydration, oral and dental care, pressure ulcers, psychotropic medications, and physical restraints
  • Review, update, and maintain resident care plans based on assessment findings
  • Participate in Medicare meetings, care plan meetings, and interdisciplinary team discussions
  • Assist the Director of Nursing with maintaining compliance with state, federal, and Americare requirements
  • Support survey readiness and participate in inspections as needed
  • Ensure medication systems meet resident needs and comply with state regulations and pharmaceutical guidelines
  • Communicate significant changes in resident condition to the Administrator and nursing leadership, including hospital transfers or other critical events
  • Assist with resident admissions, placements, and room transfers in coordination with the IDT team
  • Schedule, participate in, and maintain records for nursing staff meetings as needed
  • Review incident reports routinely and ensure appropriate documentation and follow‑up
  • Assess nursing supply and equipment needs and ensure adequate inventory levels
  • Provide on‑call support to staff as needed for nursing‑related matters
  • Perform other duties as assigned by the Administrator
