Private Duty Nurse at Advocate Home Specialty Care
  • Conduct initial assessments and regular evaluations of patients' health status and care needs
  • Develop and implement personalized care plans based on patients' individual needs and preferences
  • Administer medication, treatments, and procedures as prescribed by the primary physician
  • Monitor patients' vital signs and report any changes or concerns to the primary physician
  • Provide wound care and other nursing interventions as needed
  • Educate patients and their families on disease management, medication management, and follow-up care
  • Collaborate with other members of the healthcare team to ensure comprehensive and coordinated care for patients
  • Maintain accurate and up-to-date documentation of patient care and progress
  • Serve as a compassionate and reliable source of support for patients and their families, addressing their questions and concerns with empathy and professionalism
  • Communicate effectively with patients, families, and healthcare team members to facilitate open and clear communication channels
Qualifications:
  • Valid nursing license in the state of Kansas
  • Strong clinical skills, with excellent knowledge and understanding of nursing principles, procedures, and standards of care
  • Ability to work independently and make sound clinical judgments
  • Strong interpersonal and communication skills, with the ability to build strong relationships with patients, families, and healthcare team members
  • Compassionate, patient-centered approach to care
  • Ability to adapt to changing patient needs and prioritize tasks effectively
  • Availability to work flexible hours, including evenings and weekends
  • Valid driver's license and reliable transportation
