Private Duty Nurse at Advocate Home Specialty Care- Conduct initial assessments and regular evaluations of patients' health status and care needs
- Develop and implement personalized care plans based on patients' individual needs and preferences
- Administer medication, treatments, and procedures as prescribed by the primary physician
- Monitor patients' vital signs and report any changes or concerns to the primary physician
- Provide wound care and other nursing interventions as needed
- Educate patients and their families on disease management, medication management, and follow-up care
- Collaborate with other members of the healthcare team to ensure comprehensive and coordinated care for patients
- Maintain accurate and up-to-date documentation of patient care and progress
- Serve as a compassionate and reliable source of support for patients and their families, addressing their questions and concerns with empathy and professionalism
- Communicate effectively with patients, families, and healthcare team members to facilitate open and clear communication channels
- Valid nursing license in the state of Kansas
- Strong clinical skills, with excellent knowledge and understanding of nursing principles, procedures, and standards of care
- Ability to work independently and make sound clinical judgments
- Strong interpersonal and communication skills, with the ability to build strong relationships with patients, families, and healthcare team members
- Compassionate, patient-centered approach to care
- Ability to adapt to changing patient needs and prioritize tasks effectively
- Availability to work flexible hours, including evenings and weekends
- Valid driver's license and reliable transportation
