CLINIC - Chronic Care Manager (CCM) / Clinic Phone Nurse

Rooks County Health Center is seeking a compassionate, organized, and dedicated Chronic Care Manager (CCM)/Clinic Phone Nurse to join our team!

This position plays an important role in helping patients manage ongoing and complex health needs while supporting the practice by triaging patient phone calls. The position coordinates patient care, provides Chronic Care Management services, supports patient self-management, connects patients with appropriate healthcare services and community resources, and assists in the implementation and management of Remote Patient Monitoring (RPM) devices for patients.

The CCM/Clinic Phone Nurse works closely with physicians, fellow clinic nurses, patients, families, and other healthcare providers to develop and coordinate individualized plans of care. Responsibilities also include patient follow-up communication (transition of care and ED follow up), care management documentation, RPM set up and documentation, handling phone calls and patient triage and assisting with clinical services as needed.

Qualifications:

• Current Kansas RN or LPN license, or BSN, BSW, LMSW, or equivalent degree with applicable Kansas licensure.
• BLS/CPR Certification required
• One year of medical experience in a physician’s office or equivalent combination of training and experience preferred.
• Strong communication, organization, assessment, and interpersonal skills
• Ability to work independently while contributing effectively as part of a team
