This is a full-time hourly position. The expected work schedule is primarily Monday–Friday daytime hours with rotating/on-call responsibilities. Additional hours may occasionally be required based on resident needs, staffing, emergencies, or facility operations.

The Charge Nurse / Assistant Director of Nursing (LPN) is a hands-on nursing leadership position responsible for providing quality resident care while assisting the Director of Nursing with the daily coordination and oversight of nursing services.

This position serves as a working leader on the nursing team, providing direct resident care while also assisting with staff supervision, clinical coordination, documentation, communication, and other routine nursing-management functions. The Charge Nurse will help ensure that residents receive safe, compassionate, individualized care and that nursing practices are consistent with facility policies, applicable regulations, and established standards of care.

This position is intended to function as a working Assistant Director of Nursing, with limited administrative and management responsibilities while maintaining regular involvement in resident care. This position is expected to lead by example, maintaining a professional,respectful, and resident-centered approach. The individual in this position should be comfortable balancing hands-on nursing care with leadership responsibilities and should serve as a dependable resource for the nursing team.

The goal of this position is to provide the Director of Nursing with reliable day-to-day clinical support while maintaining a strong presence at the bedside and ensuring residents receive safe, compassionate, and consistent care.

• Provide direct nursing care to residents in accordance with their care plans, physician orders, and facility policies.
• Perform nursing assessments and monitor residents for changes in condition.
• Administer medications and treatments as ordered and within the scope of the LPN license.
• Monitor vital signs, symptoms, behaviors,wounds, nutrition, hydration, and other clinical needs.
• Communicate significant changes in resident condition to the Director of Nursing, physician, responsible party, or other appropriate healthcare professionals.
• Assist with coordination of physician visits,laboratory services, pharmacy needs, therapy services, and outside medical appointments.
• Respond appropriately to resident emergencies and initiate appropriate interventions.
• Promote resident dignity, privacy, independence,and quality of life.
• Serve as a daily clinical resource and leaderfor nursing and caregiving staff.
• Provide guidance and direction to CNAs,medication aides, and other nursing personnel as appropriate.
• Assist with assigning daily duties and ensuring adequate coverage of resident care needs.
• Monitor staff performance and adherence to established nursing procedures.
• Provide coaching, education, and feedback tostaff regarding resident care and clinical practices.
• Assist the Director of Nursing with orientation and training of new nursing employees.
• Help identify staffing concerns and communicate scheduling or performance issues to the Director of Nursing.
• Assist the Director of Nursing with theday-to-day operation of the nursing department.
• Help maintain accurate and timely clinical documentation and resident records.
• Assist with reviewing medication administration records, treatment records, care plans, progress notes, and other required documentation.
• Participate in care plan meetings and help ensure resident care plans are implemented appropriately.
• Assist with incident/accident reporting,investigation, and follow-up.
• Help monitor infection prevention and control practices.
• Assist with quality assurance and performance improvement activities.
• Assist with ensuring compliance with facility policies, state regulations, and applicable licensing requirements.
