SCHEDULE:

Generally a full-time, 40-hour-per-week schedule. Work hours may vary based on client, family, program, and organizational needs.

Purpose

The Case Manager Facilitator (Wraparound Facilitator) provides comprehensive case management, care coordination, and wraparound facilitation services for children and families enrolled in the Serious Emotional Disturbance (SED) Waiver Program.

The primary purpose of this position is to ensure youth with complex behavioral health needs receive individualized, family-driven, community-based services that promote stability, independence, overall well-being, and success across home, school, community, and other life domains.

The Case Manager Facilitator works collaboratively with youth, families, guardians, behavioral health providers, schools, foster care and child welfare agencies, insurance providers, and other community partners to develop and maintain effective, individualized plans of care. This position is responsible for coordinating services, facilitating treatment team meetings, monitoring progress, and ensuring SED Waiver services are provided and documented in accordance with applicable Medicaid, State of Kansas, and organizational requirements.

Job Functions Wraparound Facilitation and Care Coordination
  • Provide wraparound facilitation and case management services to children and families enrolled in the SED Waiver Program.
  • Coordinate individualized, strengths-based, family-driven services designed to address the youth’s behavioral health, educational, social, family, and community needs.
  • Collaborate closely with youth, parents, guardians, foster care agencies, schools, treatment providers, and other individuals identified by the family as part of the youth’s support system.
  • Promote family and youth involvement in identifying goals, strengths, needs, supports, and appropriate interventions.
Treatment Team Facilitation
  • Facilitate regularly scheduled wraparound and treatment team meetings.
  • Coordinate participation from appropriate team members, which may include mental health professionals, educators, foster care representatives, probation officers, medical providers, community agencies, family supports, and other service providers.
  • Encourage effective communication and collaboration among members of the treatment team.
  • Ensure meetings remain focused on the youth’s identified goals, strengths, needs, progress, and barriers to success.
SED Waiver Enrollment and Services
  • Coordinate referrals and assist families through the initial SED Waiver enrollment process.
  • Educate youth, families, employees, treatment team members, and external partners regarding available SED Waiver services, eligibility requirements, program expectations, and applicable procedures.
  • Assist families with navigating behavioral health and community-based service systems.
  • Coordinate authorized waiver services based on identified client needs and approved plans of care.
Plans of Care
  • Work closely with insurance and managed care providers to develop, coordinate, implement, and maintain individualized plans of care.
  • Ensure plans of care reflect the youth’s identified strengths, behavioral health needs, goals, supports, services, and desired outcomes.
  • Monitor implementation of plans of care and evaluate the effectiveness of services in achieving established goals.
