Adult & Youth Case Manager | CPST Certified

Full Time

Join a dedicated team providing community-based mental health services to adults and children facing significant challenges. As a Case Manager, you will play a vital role in enhancing clients' overall functioning while supporting them to remain in the least restrictive environments within their homes and communities across the Central Plains region.

Key Responsibilities
  • Deliver skill training focused on measurable goals determined by medical necessity and qualified professionals.
  • Provide intensive, interactive mental health services using a strengths-based, skill-building approach.
  • Collaborate with clients, families, and treatment teams to develop and maintain client-centered treatment plans.
  • Help clients understand their symptoms and explore potential solutions.
  • Assist clients and families in leveraging natural community supports.
  • Coordinate mental and physical healthcare services, facilitating communication among clients, providers, families, and community partners.
  • Perform additional duties as assigned.
Education & Experience

Minimum qualifications include one of the following: a Bachelor's degree in Social Work, Psychology, or a related field; or an Associate's degree plus two or more years of experience educating clients on self-care and daily living skills; or a combination of education and experience totaling four or more years. Preferred candidates hold a Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics, along with three to five years of professional experience in mental health or related services.

Licensure

& Training

Completion of CPST Case Management online training, KHS online training, and passing background checks including KBI, child and adult abuse registries, and motor vehicle screenings are required. Adult Case Management training must be completed within 60 days of employment. A valid driver's license, insurance, and reliable vehicle are necessary.
