As a dedicated provider of community mental health services, the Adult & Youth Case Manager plays a vital role in delivering interactive support to individuals facing significant mental health challenges. This position focuses on enhancing overall functioning and promoting independence by maintaining clients in the least restrictive environment possible, typically within their homes and communities.

• Provide skill training aligned with target goals and measurable outcomes, as determined by a Qualified Mental Health Professional.
• Deliver intensive, community-based mental health services to both children and adults using a strengths-based, skill-building approach.
• Collaborate with clients, families, and treatment teams to develop and maintain client-centered treatment plans.
• Help clients understand their symptoms and explore potential solutions.
• Support clients and families in leveraging natural community supports to foster problem-solving.
• Coordinate mental and physical healthcare services, facilitating communication among clients, providers, families, and community partners.
• Perform additional duties as assigned.

• Required:
  
  Bachelor's degree (preferably in Social Work, Psychology, or related field) OR Associate's degree plus 2+ years of experience educating clients on self-care and daily living skills OR equivalent combination of education and experience totaling 4+ years.
• Preferred:
  Bachelor's degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics; 3-5 years of professional experience in mental health or related services.

• Completion of CPST Case Management online training, KHS online training, background checks including KBI, child and adult abuse registries, and motor vehicle screening.
• Completion of Adult Case Management training within 60 days of employment.
• Valid driver's license, insurance, and reliable vehicle required.
