Responsibilities: Evaluate patient status, respond to changes per clinical protocols, document assessments and interventions, facilitate handoffs and care plan updates, and provide education to patients and families regarding discharge and care plans.
Qualifications & Skills: Minimum of 2+ years of RN experience, with strong clinical assessment, documentation, and communication skills.
