Job Overview

Position:
Case Manager III at U.S.VETS in Kapolei, HI. Salary: $55,000.00 – $60,000.00. FLSA classification:
Exempt.

Responsibilities
  • Assess veterans referred to the program, including problems, needs, or barriers in areas such as Housing, Employment, Education/Training, Financial, Sobriety Support/Addiction Treatment, Legal, Family/Social, and Medical/Psychiatric.
  • Coordinate care with VA personnel and other community partners for eligibility documentation, mental and physical health services.
  • Facilitate client intake process and compile client files ensuring all program eligibility documentation is complete.
  • Collaborate with clients to complete Individual Action Plans (IAP).
  • Provide guidance to Case Manager I & II staff on client IAPs, intervention strategies, progress notes, discharge planning, etc. during weekly program meetings and as needed.
  • Conduct crisis intervention and support Case Manager I & II staff with client issues.
  • Maintain confidential client files in a professional and timely manner, ensuring all forms are the most recently revised versions, and that all required documents are present, complete, signed by client and staff as applicable, and placed in the correct order.
  • Meet with clients as specified by program requirements, documenting progress notes in Data/Assessment/Plan (DAP) format.
  • Review and update IAPs on a monthly basis.
  • Complete timely discharge summaries.
  • Conduct home visits (project‑based units or scattered site) to provide case management services and ensure health and welfare of clients.
  • Assist in quality management activities, data collection, and preparation of reports.
  • Facilitate client life skills classes.
  • Provide specialized case management services and life skills groups/classes to veteran subpopulations as applicable (female veterans, disabled veterans, Iraq/Afghanistan veterans, etc.).
  • Conduct random drug and alcohol screenings of clients.
  • Maintain confidentiality in compliance with HIPAA.
  • Assist Program Coordinator and Clinical Director with auditing of client documentation (eligibility documentation, case notes, IAPs and IAP reviews, discharge forms).
  • Assist Program Coordinator and Clinical Director with program reporting.
  • Supervise student interns, including task assignment, reviewing process recordings, supervision sessions, evaluations, and adherence to university standards.
  • Assist in training new staff and provide training to team members.
  • Assist with urgent client issues outside business hours as needed.
  • Complete accurate and timely data entry into the HMIS system and other required databases.
  • Document client referrals for services.
  • Attend scheduled team meetings and trainings.
  • Maintain and submit timesheets, mileage logs, client expenses, and other required documentation accurately and timely.
  • Drive company vehicle for client transportation or other work‑related activities.
  • Perform other duties as required.
Additional Program‑Specific Duties
  • Transitional Housing Clients:
    • Provide case management services to help clients transition to permanent housing.
    • Ensure clients are accounted for daily via contact, bed check, or daily sign‑in logs.
    • Monitor program rent/participant fee compliance to assist clients in prioritizing housing.
    • Locate community housing units that meet clients’ needs for scattered‑site transitional housing (e.g., Transition In Place (TIP) Program).
  • Permanent Housing Clients:
    • Provide case management services to help clients obtain and maintain permanent housing.
    • For scattered‑site housing, locate community units that meet clients’ needs.
    • Ensure children and youth enrolled in school and connected to appropriate community services for families with dependent children.
  • Workforce Clients:
    • Assist with Workforce Program assessment and enrollment processes.
    • Work with clients to develop an individual employment development plan, assess and reduce barriers to employment, and help plan short‑term and long‑term employment goals.
    • Assist clients in developing cover letters, resumes, and obtaining supportive services.
