About UsA Hug Away Healthcare is a small but growing hospice agency dedicated to providing compassionate and high-quality care to our patients. We are looking for a dynamic and experienced Hospice Liaison to join our team and help us expand our services in the Houston area.
Job SummaryThe Hospice Liaison will be responsible for developing and maintaining relationships with healthcare providers, community organizations, and potential referral sources to grow our patient base. The ideal candidate will have at least 2 years of experience in a similar role, a proven track record of meeting quotas, and the ability to develop and implement strategies for growth. A vehicle may be provided, and salary will be based on quotas achieved.
Key Responsibilities- Develop and maintain relationships with healthcare providers, hospitals, nursing homes, and other potential referral sources.
- Identify and develop new accounts to increase patient referrals.
- Meet and exceed established quotas for patient admissions.
- Structure and grow designated areas of Houston to ensure consistent growth and service delivery.
- Conduct presentations and educational sessions to promote hospice services.
- Collaborate with the clinical team to ensure seamless patient care and service delivery.
- Monitor and report on market trends, competitor activities, and potential opportunities for growth.
- Ensure there are no service failures by maintaining high standards of communication and follow-up.
- Work some weekends as required to meet the needs of the agency and its patients.
- Minimum of 2 years of experience in a hospice liaison or similar role.
- Have established relationships with nurses, case managers and other sources for families we can help.
- Proven ability to meet and exceed quotas.
- Existing book of business and ability to hit the ground running.
- Strong understanding of the hospice industry, including compliance and regulatory requirements.
- Excellent communication, presentation, and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Able to quickly build territories.
- Valid driver's license and reliable transportation.
