Halcyon Home in Katy, TX is seeking a Hospice LMSW PRN to support our geriatric hospice population. You will perform social work visits, assess needs, coordinate care, and document progress in a timely manner.
License as LMSW is required with at least one year of hospice and social work experience. Strong communication, empathy, and the ability to work independently are essential. Knowledge of Homecare Homebase EMR is a plus; pay per visit is $55-$75.
