Role: Licensed Physical Therapist providing personalized in-home and senior living care to older adults, with optional telehealth services.
Responsibilities:
- Conduct evaluations
- Develop and implement evidence-based treatment plans
- Support patient mobility and independence
- Document care accurately
- Collaborate with interdisciplinary teams
- Educate patients and caregivers
- Utilize virtual care when appropriate
Qualifications & Skills:
- Valid PT license
- Degree from an accredited program
- Proficiency in EMR and computers
- Current CPR certification
- Physical ability to perform in-home care duties, including lifting and assisting patients
Key Duties:
- Deliver one-on-one therapy sessions
- Promote wellness and safety
- Support aging in place
- Adapt treatment schedules for flexibility
