The Hospice Medical Social Worker (MSW) provides emotional support, counseling, and resource assistance to patients and families facing terminal illness. The MSW collaborates with the interdisciplinary team to develop and implement individualized care plans that address psychosocial needs and promote quality of life.
Essential Functions:
Assesses and completes the Comprehensive Assessment within five (5) days of the patient’s election of hospice care to identify the patient and family’s psychosocial, financial, environmental, and community needs as evidenced by POC documentation, clinical records, IDG/T meetings, and community resource referrals.
Develop and implement individualized care plans.
Provide emotional support and counseling to patients and families.
Assist with advance directives and end-of-life planning.
Coordinate community resources and referrals.
Participate in interdisciplinary team meetings.
Document all services provided in the EMR system.
Support bereavement services and follow-up care.
Job Qualifications:
Education:
Master’s degree in Social Work (MSW) from an accredited institution. Must be licensed to practice as a social worker in the state.
Experience:
Minimum of 1 year of experience in a healthcare setting, preferably in hospice or palliative care.
Skills:
Strong communication and interpersonal skills.
Ability to work collaboratively with interdisciplinary teams.
Knowledge of community resources and support systems.
Proficiency in EMR systems and documentation.
Understanding of grief, loss, and end-of-life issues.
Environmental and
Working Conditions:
Works in patient homes, hospice facilities, and office settings.
Requires local travel and flexible scheduling.
Physical and Mental Effort:
Requires prolonged periods of driving and walking.
Must be able to manage emotionally challenging situations with compassion and professionalism.
