Family Nurse Practitioner (PRN) – Paragon Infusion Centers
Location:
Katy, TX – 21927 Clay Road Clinic
Hours:
Monday – Friday; 8:00AM – 4:00PM
- Administers ordered therapies according to prescriber order, facility protocols, INS standards, and company policy and procedures.
- Completes the admission process as directed by the Center Director.
- Performs a physical assessment on each patient as warranted, including new consults based on patient diagnosis.
- Documents all pertinent data in the patient’s medical record.
- Reports all significant changes or observations to the Center Director and follows up on any identified problems.
- Communicates effectively with other members of the IV team regarding patient status and observations.
- Maintains patient confidentiality at all times in accordance with HIPAA laws and regulations.
- Evaluates the patient’s response to therapy and documents findings in the medical record.
- Educates patients and/or family members regarding the therapy plan.
- Participates in the Quality Improvement program.
- Ensures that all medication orders are complete, appropriate, accurate, and up to date prior to treatment.
- Provides clinical coverage for nurse practitioners on PTO/LOA and may support new market launches.
- May require travel to work sites and other locations as necessary.
- MS in Nursing and a minimum of 2 years of nursing experience, or an equivalent combination of education and experience.
- Experience with intravenous therapy (IVs) required.
- Current, active, valid Nurse Practitioner license to practice within the scope of license in the applicable state.
- Active, current, valid Family Nurse Practitioner certification.
- Multi‑state licensure required if services are provided in multiple states.
- Satisfactory completion of a tuberculosis test.
- Hepatitis B vaccine or signed waiver.
- Ability to obtain credentialing with major payors in the market, including Medicare/Medicaid (preferred).
- Experience with IV insertion and maintenance (strongly preferred).
- Experience with port, PICC, and peripheral line (preferred).
- Infusion reaction management experience (preferred).
- Titration experience (preferred).
- Medication mixing and infusion experience (preferred).
