American Traveler is hiring a Home Health RN with HCHB and Oasis charting experience for a field-based role requiring on-call rotation and a valid TX or compact RN license
Details- Home Health field RN position providing in-home patient care to a geriatric population
- Day shift schedule, Monday through Friday, 5x8-hour shifts from 8:00 AM to 5:00 PM
- Average caseload of 4-6 patients per day
- Must complete charting using HCHB and Oasis while at the patient's home
- Responsible for interdisciplinary care coordination and plan of care development and oversight
- Required to work weekends as needed
- Must be open to one night of on-call coverage per week, with details discussed during interview
- Must be willing to travel up to 50 miles outside home base to cover territory
- Mileage reimbursed at IRS standard rate
- Active RN license (Texas or Compact) required in hand at time of consideration
- Current BLS certification required
- Minimum 1 year of Home Health experience required
- Minimum 2 years of overall clinical experience as a nurse required
- HCHB (Homecare Homebase) and Oasis charting experience required
- Valid driver's license required
- Must have reliable transportation and insurance
- Non-smokers required due to patient sensitivity to smoke exposure
- Administer skilled nursing care to patients from diverse populations in a variety of home environments
- Coordinate care with patients, families, certifying providers, and interdisciplinary team members
- Perform comprehensive patient assessments and collaborate on plan of care development
- Scrubs of any color required for dress code
- First-time travelers are welcome to apply
- Candidates are accepted from within a 50-mile radius of the home base office
- CHPN certification is preferred but not required
