Position Title

Mason Creek Transitional Care of Katy – Licensed Social Worker (Full Time).

Location:

21727 Provincial Boulevard, Katy, TX 77450. Must hold the Texas Social Worker License.

Responsibilities
  • Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of the facility.
  • Assist in the development, administration, and coordination of department policies and procedures.
  • Keep abreast of current federal and state regulations, and professional standards, and make recommendations on changes in policies and procedures to the Director or Administrator.
  • Review department policies and procedures at least annually, and participate in making recommended changes.
  • Assist in developing and implementing policies and procedures for identifying medically related social and emotional needs of residents.
  • Interview residents and families to obtain social history.
Qualifications
  • Bachelor of Social Work (BSW) degree.
  • Long‑term care experience or casework experience preferred.
  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public.
  • Leadership ability and willingness to work harmoniously with and supervise other personnel.
  • Patience, tact, cheerful disposition, and enthusiasm; willingness to handle residents at any maturity level.
