Salary Range: BU29 | SR23

Salary Rate: $ (Annual Salary) based on the FTE

Job Duties: The Case Management department provides outcome‑oriented patient care, appropriate length of stay, resource utilization based on case types, and coordination of an integrated system of settings, services, health care practitioners, and care levels that make up a continuum of care. The Registered Professional Nurse V (Case Management Lead) assists the director with scheduling of staff, budget monitoring, PAS reports, and quality improvements.

This role also performs regular case manager duties: outcome‑oriented patient care, promotion of integration and coordination of clinical services, monitoring resource use, supporting collaborative practice, ensuring continuity of care, and enhancing patient and provider satisfaction. May be required to work shifts, weekends, and holidays.

Minimum Qualification Requirements

Basic

Education:

Graduation from an accredited school of nursing. A Bachelor’s Degree from an accredited college or university is desirable.

Experience:

  • Minimum 2½ years of professional nursing experience, with at least 1 year comparable to a Registered Professional Nurse IV supervising nurses and managing patient care and activities in a ward or clinical area.
  • Preferably 2 years of hospital experience demonstrating administrative, supervisory, teaching, and nursing abilities.
  • One‑half year of supervisory experience in planning, directing, assigning, reviewing, training, and developing staff.
  • A master’s degree in nursing may substitute for one year of experience.

License: Current license to practice as a professional nurse in Hawaii. Possession of a motor vehicle operator’s license may be required for certain positions.

Certification: Certified professional in Utilization Review or Case Management preferred.

Knowledge and Abilities:

  • Knowledge of nursing theory and practice, techniques, procedures, literature, and current trends.
  • Ability to apply theory in planning and delivering care, recognize and evaluate symptoms, and take appropriate actions.
  • Skills in establishing and maintaining working relationships with medical staff, administration, nursing, and other departments/agencies.
  • Knowledge of JCAHO requirements and other regulatory bodies.
  • Proficiency in supervision principles and practices.
  • Strong communication, teamwork, management, leadership, decision‑making, interpersonal, and communication skills.
