Our Home Health & Hospice RN Case Managers are compassionate healthcare professionals who provide skilled nursing care, coordinate services, and support patients and families across a variety of healthcare needs. Whether helping patients recover and maintain independence through home health services or providing comfort and guidance during the hospice journey, they combine clinical expertise with empathy to deliver exceptional, patient-centered care in the home setting.

They build trusting relationships, advocate for their patients, and help ensure the highest quality of care every step of the way.

We are seeking multiple Full-Time Home Health & Hospice RN Case Managers to serve Grand Island and the surrounding areas. Qualified candidates will possess:

• Current RN licensure
• Current CPR certification
• Strong clinical assessment and care coordination skills
• Excellent communication and interpersonal abilities
• A positive, caring, and professional attitude
• A passion for providing high-quality home health and hospice care
• Strong personal integrity and commitment to patient-centered service
