Ritemed is seeking a Nurse Practitioner to provide on-site clinical services at Alaris Health at Kearny.

This is a facility-based role within a skilled nursing environment. The Nurse Practitioner will manage post-acute and long-term care residents while working closely with physicians and facility leadership to support strong clinical outcomes.

Key Responsibilities

Provide comprehensive medical care to residents within the nursing home
Perform assessments, follow-ups, and manage acute changes in condition
Collaborate with physicians and nursing leadership
Participate in care planning and interdisciplinary meetings
Ensure timely and accurate clinical documentation
Support quality initiatives and rehospitalization reduction efforts

Qualifications

Active New Jersey Nurse Practitioner license
Minimum of 6 months NP experience required
Nursing home experience required
Strong clinical assessment and decision-making skills
Ability to work independently within a structured clinical model
