We are seeking an experienced and motivated Nurse Practitioner (NP) to provide high-quality medical care within Skilled Nursing Facilities (SNFs). - Earn$150,000–$180,000+ annually
- Choose W2 or 1099 employment
- Monday–Friday schedule
- No nights, weekends, or major holidays

Long-term career stability and growth
- Conduct comprehensive patient evaluations and assessments
- Manage acute and chronic medical conditions
- Perform routine patient rounds within Skilled Nursing Facilities
- Develop and implement treatment plans
- Order, review, and interpret diagnostic testing and laboratory results
- Collaborate with physicians, nursing staff, therapists, and interdisciplinary care teams
- Maintain accurate and timely EMR documentation
Participate in care planning and quality improvement initiatives
- Active Nurse Practitioner license
- DEA license (or ability to obtain)
- Experience in Skilled Nursing, Post-Acute Care, Long-Term Care, Geriatrics, Internal Medicine, Family Practice, or Hospital Medicine
- Strong clinical assessment and diagnostic skills
Ability to work independently while collaborating with a multidisciplinary team
- Previous SNF or Post-Acute Care experience an an RN or Nurse Practitioner
- Experience managing medically complex patients
Strong communication and leadership skills
- $150,000–$180,000+ annually
- Compensation based on experience and qualifications
