Certified Occupational Therapy Assistant (COTA) – Skilled Nursing Facility (SNF)
We are seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join a skilled nursing facility team providing short-term rehabilitation and long-term care services. This role offers the opportunity to work with a primarily geriatric population focused on restoring functional independence, improving safety, and enhancing quality of life.
The COTA will implement individualized treatment plans developed by the supervising Occupational Therapist and play a key role in helping residents regain strength, mobility, and confidence following illness, surgery, or hospitalization.
Key Responsibilities:
Provide skilled occupational therapy interventions under OT supervision
Assist residents with ADL retraining including dressing, bathing, grooming, toileting, and feeding
Address functional mobility, balance, endurance, and safety awareness
Implement fall prevention strategies and adaptive techniques
Train patients in use of adaptive equipment and compensatory strategies
Accurately document daily treatment sessions and progress in EMR
Collaborate with interdisciplinary team including nursing, PT, SLP, and case management
Maintain productivity standards while ensuring high-quality patient-centered care
Ideal Candidate:
Active COTA license in practicing state
SNF or geriatric experience preferred
Strong time management and clinical reasoning skills
Comfortable managing a varied caseload of short-term rehab and long-term residents
