Role Overview
A full-time Advanced Practice Provider (nurse practitioner or physician assistant) provides primary and urgent care to residents in long-term care facilities. This float position covers two centers in Keene, NH, offering work-life balance with no on‑call or weekend duties. The role involves collaborating with center leadership and medical staff to deliver high-quality healthcare aligned with the organization's mission to improve patient outcomes and quality of life.
ResponsibilitiesCollaborate with center leadership and medical directors, deliver patient‑specific care, and work towards facility goals. Provide compassionate, high‑quality health services to the geriatric population, supporting the team in achieving healthcare excellence.
Qualifications & SkillsMust be a licensed registered nurse with a master's degree in nursing (preferably with an Older Adult or Family Nurse Practitioner focus) or a certified Physician Assistant with NCCPA certification and state licensure. Prior experience with adults or geriatrics is preferred. Unrestricted licenses and DEA registration are required where applicable.
