Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN)

Provide licensed nursing care to patients in long-term care or skilled nursing facility settings, ensuring their health and well-being. Deliver quality patient assessments, medication administration, and vital sign monitoring under the supervision of healthcare professionals.

• Administer prescribed medications and treatments accurately according to established protocols.
• Conduct patient assessments and monitor vital signs regularly.
• Document patient care activities and updates in medical records.
• Collaborate with healthcare team members to develop and implement care plans.
• Follow facility policies regarding infection control, safety, and patient confidentiality.

• Valid LVN or LPN license required.
• Proven experience in LTC or SNF environments preferred.
• Ability to follow detailed instructions and document care accurately.
• Strong communication and interpersonal skills.
• Must have current CPR certification and be able to pass facility-required background checks.
