LVN ​/ LPN – LTC​/SNF

Nursing Position in Long-Term Care or Skilled Nursing Facility

Provide comprehensive nursing care to residents in a Long-Term Care or Skilled Nursing Facility setting. Ensure patient wellbeing through medication administration, assessments, and collaboration with the healthcare team.

Responsibilities:

• Administer prescribed medications and treatments accurately and safely.
• Conduct patient assessments and monitor vital signs regularly.
• Document patient care activities in compliance with facility policies.
• Collaborate with healthcare team to develop and update care plans.
• Educate residents and families about health status and care procedures.
• Maintain compliance with safety and Infection Control protocols.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in LTC/SNF environments preferred.
• Ability to perform accurate assessments and interventions.
• Strong communication and documentation skills.
• Flexibility to work various shifts as needed.
• Compliance with COVID-19 vaccination requirements or approved exemptions.
