LVN ​/ LPN – LTC​/SNF

Nursing Care In Long-Term Care Or Skilled Nursing Facilities

Provide nursing care to patients in long-term care or skilled nursing facilities as an LVN or LPN. Ensure patient comfort, safety, and adherence to care plans within a team environment.

Responsibilities:

• Administer medications and treatments accurately according to physicians’ orders.
• Monitor patient vital signs and observe for any changes in condition.
• Document patient care activities and report pertinent information to the healthcare team.
• Assist with daily living activities and provide emotional support to patients.
• Follow facility protocols and maintain compliance with health and safety standards.

Qualifications:

• Current LVN or LPN license in the state of practice.
• At least 1 year of experience in LTC or SNF settings.
• Ability to effectively communicate with patients, families, and staff.
• Knowledge of medication administration, wound care, and infection control procedures.
• Must be able to work flexible shifts, including weekends and holidays.
