LVN ​/ LPN – LTC​/SNF

Job Summary:

Seeking a dedicated LVN/LPN to provide quality nursing care in a Long-Term Care/SNF environment. The ideal candidate will support resident needs and collaborate with healthcare teams to ensure optimal patient outcomes.

Responsibilities:

• Administer prescribed medications and treatments accurately and timely.
• Monitor and record residents’ vital signs and clinical progress.
• Assist with activities of daily living and provide emotional support to residents.
• Collaborate with medical staff to develop and implement care plans.
• Ensure compliance with facility policies, safety procedures, and regulatory standards.

Qualifications:

• Valid LVN or LPN license in the state.
• Minimum of one year of experience in long-term care or skilled nursing facilities.
• Strong understanding of nursing principles and resident care protocols.
• Ability to communicate effectively with residents, families, and team members.
• Flexibility to work various shifts as scheduled.
