Job Summary:

Provide skilled nursing care to patients in long-term care or skilled nursing facility settings. Ensure adherence to care plans, maintain patient safety, and promote optimal health outcomes.

Responsibilities:

• Administer medications and treatments in accordance with prescribed orders.
• Monitor patient vital signs and condition, promptly reporting any changes.
• Assist with activities of daily living and provide compassionate patient support.
• Document patient care activities accurately and timely.
• Follow infection control protocols and uphold safety standards.
• Collaborate with healthcare team members to develop and update care plans.

Qualifications:

• Hold an active LVN or LPN license issued by the state.
• Demonstrate?? one year of recent experience in LTC or SNF settings.
• Complete a medical clearance, including updated COVID-19 vaccination documentation or valid exemption.
• Possess strong clinical skills and attention to detail.
• Exhibit excellent communication and teamwork abilities.
