A community service provider in Keene, NH is seeking a compassionate and organized Case Manager to join its Child & Family Team. This role provides community-based case management and care coordination to children and families experiencing behavioral health challenges and complex psychosocial needs.
Key Responsibilities- Provide case management and care coordination services to children, adolescents, and families.
- Connect clients and families with behavioral health, medical, educational, housing, financial, transportation, and community resources.
- Coordinate care with hospitals, schools, healthcare providers, residential programs, and community agencies.
- Assist families with benefits, applications, transportation, and other resource needs.
- Support clients with treatment goals, daily living skills, coping strategies, and community functioning.
- Monitor client needs and respond appropriately to clinical concerns and crises.
- Participate in treatment team meetings, supervision, and case consultations.
- Maintain timely and accurate documentation in the electronic medical record.
- Advocate for clients and families while promoting independence, self-determination, and recovery.
- Bachelor’s degree in human services or related field.
- At least one year of experience working with children, adolescents, and families.
- Strong skills in advocacy, outreach, assessment, crisis intervention, and supportive counseling.
- Excellent communication, organization, and interpersonal skills.
- Community mental health or targeted case management.
- Residential or psychosocial rehabilitation services.
- Medicaid documentation and Targeted Case Management.
- Care coordination with hospitals and community providers.
- Trauma-informed and recovery-oriented care.
