A community service provider in Keene, NH is seeking a compassionate and organized Case Manager to join its Child & Family Team. This role provides community-based case management and care coordination to children and families experiencing behavioral health challenges and complex psychosocial needs.

Key Responsibilities
  • Provide case management and care coordination services to children, adolescents, and families.
  • Connect clients and families with behavioral health, medical, educational, housing, financial, transportation, and community resources.
  • Coordinate care with hospitals, schools, healthcare providers, residential programs, and community agencies.
  • Assist families with benefits, applications, transportation, and other resource needs.
  • Support clients with treatment goals, daily living skills, coping strategies, and community functioning.
  • Monitor client needs and respond appropriately to clinical concerns and crises.
  • Participate in treatment team meetings, supervision, and case consultations.
  • Maintain timely and accurate documentation in the electronic medical record.
  • Advocate for clients and families while promoting independence, self-determination, and recovery.
Qualifications
  • Bachelor’s degree in human services or related field.
  • At least one year of experience working with children, adolescents, and families.
  • Strong skills in advocacy, outreach, assessment, crisis intervention, and supportive counseling.
  • Excellent communication, organization, and interpersonal skills.
Preferred Experience
  • Community mental health or targeted case management.
  • Residential or psychosocial rehabilitation services.
  • Medicaid documentation and Targeted Case Management.
  • Care coordination with hospitals and community providers.
  • Trauma-informed and recovery-oriented care.
