Outpatient Physical Therapist – Full Time – Keller Oaks Healthcare Center, Keller, TX

Keller Oaks Healthcare Center is a post‑acute care facility offering long‑term, short‑term, and geriatric outpatient services. We seek a dedicated Outpatient Physical Therapist for our mobile Skilled Nursing rehabilitation program. This position offers paid travel time.

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans that adhere to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

• Physical Therapist license is required.
• Travel and reliable transportation required.
• Open to Physical Therapists at all experience levels; new graduates are welcome.
