Job Summary

The Health Educator is responsible for the facilitation of wellness workshops, events and activites in KIT and the surrounding community. This position will partner with other Wellness team members, KIT departments, and community health programs to develop and deliver holistic, culturally-honoring wellness programming. The Health Educator should facilitate both one-on-one and group education and intervention programs utilizing motivational interviewing and evidence based practices to help un'ina (patients) meet their health and wellness goals.

Functions

Assists the Accreditation and Policy Manager in the following tasks:

  • Research and maintain library of best practice curricula and resources
  • Implement and evaluate evidence based chronic disease management, tobacco prevention and cessation, and other common health promotion programs
  • Provide one-on-one tobacco cessation and/or health coaching for supporting un'ina to reach their behavior change goals
  • Utilize health promotion theories and models to collaborate with Wellness team to develop, present and evaluate health and wellness education and promotion workshops for un'ina and staff
  • Effectively utilize the electronic health record to document health and wellness education activities for collaboration with integrated care team
  • Coordinate health and wellness promotion activities with departments across the Kenaitze Indian Tribe, including but not limited to Behavioral Health, Early Childhood Center, Yaghanen Youth and Elders programs
  • Participate in care team training and learning activities
  • Participate with the local tobacco coalition to support the outreach goals as outlined in the Tobacco Prevention and Control Grant, develop and implement evaluation strategies and submit reports as required
  • Complete monthly reports to include data tracking and program reflection and evaluation for both internal and grant reporting use as required
  • Maintain knowledge of current health disparities that impacts un'ina
  • Ensures compliance with Tribal, federal, state, and local employment laws and regulations, Tribal policies and TERO ordinance
