The Social Worker is a key member of the interdisciplinary team, responsible for assessing psychosocial symptoms of patients and families, developing outcomes, and providing psychoeducation, supportive counseling, and casework interventions. Key duties include:

• Conducting bio-psycho-social-spiritual assessments.
• Offering individual and family counseling.
• Referring to community resources.
• Maintaining accurate patient documentation.
• Participating in interdisciplinary care planning and team meetings.
• Providing bereavement care and psychosocial education.

Qualifications include a Master’s in Social Work, at least one year of healthcare experience, and relevant state licensure. Strong communication skills and the ability to work in a team are essential.
