- Complete POE and address care gaps during patient encounters.
- Accurately document patient data, actions, and responses to treatment.
- Assist with procedures and monitor patient responses.
- Actively engage patients, families, and caregivers in care coordination.
- Lead the nursing response to medical emergencies.
- Monitor electronic messaging related to patient or provider requests.
- Initiate and monitor IV therapy and medication administration.
- Assess health education needs and provide counseling.
- Conduct follow-up calls to patients for care and outreach.
