LPN - PRN - Home Health

This role is for a dedicated LPN (Licensed Practical Nurse) providing home health care on a PRN basis, primarily serving post-hospital or inpatient patients with varying acuity levels.

• assessing and managing patient needs following physician orders under RN supervision
• performing routine nursing procedures
• coordinating care with the healthcare team
• educating patients and caregivers on self-care and support
• accurately documenting care activities
• participating in staff meetings, in-services, and continued education
• serving as a preceptor for new staff

• minimum one year of medical/surgical nursing experience, preferably in acute care
• current LPN license and BLS certification
• high school diploma or GED
