Job Summary:

Provide comprehensive physiotherapy services to patients in a home health setting, focusing on improving mobility, strength, and functional independence through personalized treatment plans. Adapt clinical techniques to meet individual patient needs while maintaining compliance with agency standards.

Responsibilities:

• Evaluate patients’ physical conditions and develop tailored care plans to address mobility and rehabilitation goals.
• Administer therapeutic interventions, including manual therapy, exercises, and modalities.
• Monitor patient progress and adjust treatment plans accordingly to optimize recovery outcomes.
• Document patient assessments, interventions, and progress accurately within the electronic medical records system.
• Educate patients and caregivers on exercises, safety, and injury prevention strategies.
• Communicate effectively with the interdisciplinary team to coordinate care and ensure continuity of services.

Qualifications:

• Hold an active state license as a Physical Therapist.
• Possess at least 1 year of recent adult home health experience.
• Demonstrate proficiency in OASIS documentation and utilization.
• Be proficient in the use of Home Care Home Base EMR or willing to complete retraining if necessary.
• Be reliable with transportation, current driver’s license, and agency-required liability insurance.
• Have current CPR/BLS certification.
