Position: Clinical Care Coordinator - POPULATION HEALTH

Description

The Clinical Care Coordinator, in alignment with the goals of PCMH (Patient Centered Medical Home),will be primarily responsible for panel management within primary care. The Clinical Care Coordinator is responsible for establishing and maintaining multiple working relationships with; providers, internal and external care teams, front office assistants and primary care core leaders to ensure optimal quality health care.

Required Qualifications
  • Upon hire:
    National Provider BLS - American Heart Association
  • Within 60 days of hire:
    Washington Medical Assistant Registered
  • Office experience including, customer service, computers, Microsoft Office software (Excel, Publisher and Power Point), and patient interaction experience.
Preferred Qualifications
  • 2 years Healthcare experience.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
