Job Summary:

Seeking a qualified Physical Therapy Assistant to provide skilled therapy services to patients in a home health setting. The role involves making home visits, documenting patient progress, and collaborating with healthcare teams to enhance patient recovery.

Responsibilities:

• Administer therapeutic treatments and interventions to patients in their homes.
• Document patient assessments, progress notes, and treatment plans accurately and timely.
• Communicate effectively with supervising therapists, patients, and family members.
• Travel to multiple patient homes within the designated territory, maintaining reliable transportation.
• Adhere to all clinical and safety protocols, including CPR and BLS certifications.
• Participate in periodic office meetings and training sessions as required.

Qualifications:

• Valid Physical Therapy Assistant license in good standing before start date.
• Minimum of 1 year of recent home health or equivalent clinical experience.
• Strong knowledge of home health care protocols and documentation standards.
• Proficiency in electronic medical records systems (e.g., HCHB or similar).
• Excellent communication and interpersonal skills.
• Ability to travel up to 75 miles and navigate efficiently within assigned territory.
