Physical Therapist In Home Health
Join our team as a Physical Therapist in Home Health, where you'll provide essential care to patients in their homes.
What You'll Be Doing- Conduct home visits for patient assessments and treatments
- Participate in periodic office meetings
- Manage a caseload of 5-10 patients per day
- Document patient progress and activities
- Travel between patient homes, with mileage reimbursed
- Current CPR and BLS certifications
- At least 1 year of adult home health experience
- Reliable transportation with a current driver's license
- Agency-required liability insurance
- Experience with HCHB EMR systems
- Willingness to travel up to 75 miles from home clinic
