Physical Therapist In Home Health
Join our team as a Physical Therapist in Home Health, where you'll provide essential care to patients in their homes.
What You'll Be Doing- Conduct home visits for patient assessments and therapy
- Participate in periodic office meetings
- Manage a caseload of 5-10 patients per day
- Document patient progress and activities
- Travel up to 75 miles from the home clinic
- At least 1 year of recent Adult Home Health experience
- Current driver's license
- Agency-required liability insurance
- BLS/CPR certification
- Reliable transportation
- Experience using HCHB EMR systems
