The Mental Health Clinician plays a vital role in supporting youth and families enrolled in the WISe program by providing comprehensive, community-based behavioral health services that promote safety, stability, and long-term well-being. As a core member of a multidisciplinary care team, this position advances LCSNW's mission by delivering family-driven, trauma-informed care, coordinating services across systems, and empowering youth and families to achieve meaningful, lasting outcomes.

• Maintain an active caseload of up to 25 youth and families enrolled in the WISe program, providing ongoing clinical services and support throughout their participation.

• Provide individual, group, and family therapy using culturally responsive, trauma-informed, and evidence-based practices.

• Partner with youth, families, and WISe team members to assess strengths and needs, and develop, implement, and update individualized treatment plans aligned with the Cross Systems of Care Plan.

• Serve as a key clinical member of the WISe team, supporting coordination, collaboration, and communication across youth, families, and service providers.

• Identify and address barriers to engagement, collaborating with youth, families, and team members to support participation in services.

• Meet productivity and documentation expectations for the role, including timely completion of progress notes, assessments, treatment plans, and other required clinical documentation.

• Provide community-based services, including travel to family homes and community locations, with optional participation in a rotating 24/7 on-call schedule.
